Client Consent and Waiver

I acknowledge that I am voluntarily receiving a spray tanning service from The Sun Lab LLC. I understand and agree to the following:

  1. Nature of the Service:

  • The spray tanning process involves the application of a tanning solution to my skin, which may vary in results depending on my skin type, preparation, and aftercare.

  1. Potential Risks:

    • I am aware that there may be potential risks, including but not limited to allergic reactions, uneven results, or staining of clothing and belongings.

  2. Preparation and Aftercare:

    • I have been informed of the required skin preparation and aftercare steps to ensure the best results and minimize potential issues.

    • Failure to follow these instructions may result in suboptimal outcomes, for which The Sun Lab LLC is not responsible.

  3. Release of Liability:

    • I release The Sun Lab LLC from any liability for adverse reactions, dissatisfaction with the results, or damages caused by improper preparation or aftercare.

Service Agreement

This agreement is made between The Sun Lab LLC, Gabrielle Burke, and the client.

  1. Scope of Services:

    • The Service Provider agrees to provide the following service(s): Spray tan application.

  2. Pricing and Payment:

    • The total cost of the service is $75 for a single tan, if more clients are involved, the price is agreed upon before arrival.

    • Payment is due at the time of service.

  3. Cancellation Policy:

    • Cancellations made within 24 hours of the scheduled appointment may be subject to a fee.

  4. Client Responsibilities:

    • The Client agrees to:

      • Be present and prepared at the scheduled location and time.

      • Follow all preparation and aftercare instructions provided by the Service Provider.

  5. Refunds and Rescheduling:

    • Refunds are not provided.

    • Rescheduling must be requested at least 24 hours in advance.

Model Release Agreement

I grant permission to The Sun Lab LLC to use my photograph(s) taken before, during, or after my spray tanning session for promotional and marketing purposes, including but not limited to social media, websites, and print materials.

I understand that:

  • My images will be used in a professional manner.

  • I will not receive compensation for the use of these images.

I retain the right to revoke this consent at any time by providing written notice to The Sun Lab LLC.

Terms of Service

By booking a service with The Sun Lab LLC, you agree to the following terms:

  1. Booking Policy:

    • All bookings are subject to availability and must be confirmed by The Sun Lab LLC.

  2. Privacy Policy:

    • Client information will be kept confidential and used only for service purposes.

  3. Limitations of Liability:

    • The Sun Lab LLC is not liable for adverse reactions or dissatisfaction resulting from failure to follow preparation and aftercare instructions.

By clicking the checkbox available during booking, I confirm that I have read and understood this waiver and the above terms and consent to the spray tanning service.
